New Hampshire Adult Family Care (AFC) Program

The New Hampshire Medicaid-funded Adult Family Care (AFC) Program under the CFI Waiver allows individuals who need daily support to receive care from someone they know and trust, often a family member or friend. This program ensures the care recipient can remain in a safe, familiar home environment while caregivers are supported and recognized for their dedication.

Who qualifies as a caregiver under Medicaid rules?

Client Eligibility
  • Be a resident of New Hampshire.
  • Be 18 years of age or older.
  • Need assistance with at least one or more activities of daily living (ADLS).
  • Have the CFI Medicaid Waiver in order to receive this service.
  • Reside in the same home as the caregiver.
Caregiver Eligibility
  • Be at least 21 years of age.
  • Can be a family relative, spouse, or friend, but not a Legal guardian.

What are ADLs?

Caregivers are compensated for helping with activities for daily living (ADLs). They include:

  • Feeding
  • Bathing/Personal Hygiene
  • Dressing
  • Using the Toilet
  • Ambulating
  • Transportation

How to Become a Paid Caregiver for a Loved One in New Hampshire

With the Family Support Services AFC Program, you can care for your loved one at home and receive financial support for the important work you do. Getting started is simple; we walk with you every step of the way.

Meet the Requirements

Make sure you and your loved one qualify for the AFC Program. Once confirmed, you’ll enroll with Family Support Services to begin the process.

In-Home Visit

One of our registered nurses will visit your home to meet with you and your loved one. Together, we’ll review care needs and create a plan that works best for both of you.

Enrollment & Agreement

As a caregiver, you’ll receive a formal agreement with Family Support Services. This makes your role official and allows you to begin receiving compensation

Compensation & Ongoing Support

Once enrolled, you’ll start receiving monthly, tax-free compensation for providing care. You’ll also have the ongoing support of a dedicated nurse, who provides regular visits, expert guidance, and encouragement
