Ohio’s Structured Family Caregiving (SFC) program

Ohio’s Structured Family Caregiving (SFC) program allows seniors who need daily support to remain safely in the comfort of their own homes—while receiving care from someone they trust. Through the PASSPORT Medicaid Waiver, eligible clients can choose a family member or close friend to provide non-medical care and 24-hour supervision. Caregivers receive tax-free compensation through an approved agency.

SFC promotes independence, safety, and dignity while supporting family members who step up to provide care.

Shared Living Services (DDS)

In collaboration with the Department of Developmental Services (DDS), we offer Shared Living Services, a program that allows individuals with disabilities to live in a supportive home environment. A trusted caregiver offers personalized attention and support, helping individuals lead fulfilling lives in a nurturing, family-like setting.
