Who Can Become an AFC Caregiver?
You may be able to become an AFC caregiver if you:
- Are a trusted family member or friend of an eligible adult receiving care.
- Are willing and able to provide daily support based on the person’s needs and care plan.
- Can meet the program’s caregiver requirements, including applicable screening, training and enrollment steps.
- Are prepared to participate in ongoing care coordination and follow the approved care plan.
