Who Is Eligible for the AFC Program Through Medicaid?
You may qualify for AFC if you:
- Are 18 or older and meet the program’s age requirements.
- Have Medicaid coverage and meet the applicable Medicaid eligibility requirements.
- Need help with daily activities, such as bathing, dressing, eating, mobility or personal care.
- Require ongoing support because of a medical condition, disability or age-related needs.
- Can receive care safely at home with support from an eligible family member or friend.
- Have an appropriate caregiver who is willing and able to provide the care you need.
