Indiana’s Structured Family Caregiving (SFC) Program, funded by Medicaid, allows individuals who need daily support to receive care from someone they know and trust—often a family member or close friend. Instead of moving into a facility, clients can continue living in a safe and familiar home environment while being cared for by a loved one.

Who qualifies as a caregiver under Medicaid rules?
Client Eligibility
- An Indiana resident
- Qualify for the Pathways for Aging Waiver, which is for individuals who are 65 or older, or H&W / TBI Waiver if between 18 and 64 with a recognized disability.
- Need assistance with at least two or more activities of daily living (ADLS).
Caregiver Eligibility
- Must be 18 years of age.
- Must reside in the same home as client
- Can be a family relative, friend, or spouse, but not a Legal guardian.
What are ADLs?
Caregivers are compensated for helping with activities for daily living (ADLs). They include:
- Feeding
- Bathing/Personal Hygiene
- Dressing
- Using the Toilet
- Ambulating
- Transportation
